
AP reported that the Occupational Safety & Health Administration has proposed penalties of USD 46,935 for Quality Steel Corporation in Cleveland, where a worked died in 2010 in a forklift accident.
OSHA said in a news release that the propane tank manufacturer was cited for 19 safety violations.
OSHA said that the safety violations included failing to ensure the forklift operator was wearing a seat belt, not conducting required frequent and periodic inspections on lifting equipment and allowing forklift modifications to be made that were not approved by the manufacturer.
Authorities said that Mr Robert Mason was killed in October 2010 when the weight shifted on the forklift, causing it to turn over and crush him.
The company has 15 days to contest the citations and proposed fines.
