Peoria County Coroner Mr Johnna Ingersoll said that maintenance worker David Hezlet of Cuba was pronounced dead at OSF Saint Francis Medical Center.

Mr Ingersoll said that Mr Hezlet was driving a golf cart through the plant when it veered off path and collided with a horizontal I beam.

Investigators are trying to figure out what caused the cart to veer. An autopsy is planned.

Mr Hezlet is the second Keystone employee to die at work in July 2011.

Authorities say 40 year old Mr John DeCanter of Manito died July 20th 2011 after suffering a heart attack and collapsing in the plant's shower facility.
